Check these in order:

1. No usage yet — the table only lists active reasons, and a reason that has never been chosen still appears with 0%; if the table is completely empty, you may have no active reasons.
2. Permission quirk — in the current version the page itself needs List Subscription Reasons, but the data request behind it is checked against the List Zipcodes permission. A role without List Zipcodes sees the page but the table never fills. Grant that permission to the role.
3. Kitchen location — the page requires an active kitchen location to be selected.
4. Search — clear the search box; it filters by reason name.
