The Extra Meals section with Add extra meal appears only when all of these are true: Client Config > Order: Subscription > Meal Plan: Allow Extra Meals is Yes; the subscription's General Settings > Can Add Extra Meals is Yes; the subscription's Lock Menu is No; and, for account members, the member is allowed to buy extras. Extra meals are charged at their normal price on the meal-selection order. Portion upgrades are a separate switch: Meal Plan: Allow Portions Upgrade.
