They're per-user opt-outs for marketing/transactional SMS and email respectively. Turning either one off records which admin or staff member last changed it on the account (not a timestamp). Sprwt's marketing tools — email campaigns, SMS, and the Klaviyo/automation flows — all check these flags first and skip a user whose flag is off before sending.
