How do I put a form on a page (embed it)?

  1. Build the form in Form Builder.
  2. Open the page in Website > Pages, click + Add Module and choose Sprwt Lead Forms (category Forms) — or Sprwt Lead Forms with video to show a video/image beside it.
  3. Edit the module: pick the form under Form, set Send Submitted Data By Email (on by default) so each submission is emailed to you, optionally Full Size and Title Color. The video variant adds Title/Subtitle styling, a background image and video settings.
  4. Save. Submissions from this page are recorded with the page name in Results.

A form set to not Show to Registered Users is hidden from logged-in customers on the page (admins still see it). Forms cannot be embedded on external websites in the current version — link visitors to the page instead.
