1. Open Reports > Sales Reports > Conversion and set the dates to the period you care about.
2. Click the Canceled Subscriptions tile (View more).

The pop-up lists each subscription cancelled in that period with Id. (link to the subscription), Name, Email, Phone, Plan, Reason (the option chosen in your cancel funnel), Group and Cancelled At. Export it with the PDF/Excel buttons. The Paused Subscriptions tile works the same way with a Paused At column.
